Transitional Care Management is owned by Dr. Reynolds. TCM provides substance use, mental health, and MAT (Medication Assisted Treatment) services to individuals recently released from Federal prison. Services are provided in Dallas, Tyler, and Austin, Texas. It is an honor to provide these services and help clients and their families with their transition into the community.

We provide individual, family, and group counseling, as well as psychiatric services for medication management.

We have wonderful therapists, psychiatrists, and administrative staff that assist the clients and communicate with the residential re-entry centers to make the transition as easy as possible.
